Influenza (Flu) Vaccination
Bay Area Community Health is now offering flu vaccines to patients and community members.
Flu vaccines are more important than ever this year. Get your vaccine to protect yourself and your family from the flu.
Walk-ins are accepted; however, registration is highly recommended to help reduce your wait time and to ensure availability
If under the age of 18, you must have oral or written consent by a parent/ guardian.
A parent/ guardian must be available by phone for any questions.
On day of flu vaccination:
Consider wearing a short-sleeved shirt, or wear a short-sleeved shirt under a sweater or jacket that can be easily removed
Present government-issued ID, if available
Health Insurance ID card, if available
Complete the registration packet below
